Special Dietary Needs Information

The Food Service Department is planning for the 2024-2025 school year. Any student needing a special diet due to a disability must have the “Medical Plan of Care for School/Site Food Service” sent in to the Food Service Department to receive meal modification. Per federal guidelines, this form requires a written doctor’s statement that must provide the following information and must be signed by both the physician (Nurse Practitioner, Physician’s Assistant, Registered Dietitians, and Dentist are also acceptable) and parent/guardian.

·         Medical disability or diagnosis

·         Food or foods not allowed

·         Food or foods to be substituted for the foods not allowed

·         Name, address and phone number of the physician

If your child’s special diet request is NOT life-threatening the Food Service Department is not required to make menu substitutions. Food substitutions will be made at the discretion of the Food Service Department for students who do not have a life-threatening allergy or disability but are medically certified as needing a meal modification.

Below you will find the Medical Plan of Care for School/Site Food Service and the Definition of Children with Disabilities and Special Dietary Needs. We ask that you please have this filled out by the doctor and return to the Food Service Department and your school nurse.
