1095-C Request
Trinity Area School District will issue 1095-C tax forms only upon individual employee request, in accordance with the Paperwork Burden Reduction Act of 2024. Employees may request an electronic copy of their 1095-C tax form by using one of the provided options and must include name, current address, and the tax year requested:
